Congratulations on your decision to retire!
Now that you have reached this milestone, let’s look at the retirement process:
1. You must complete the retirement application link which includes:
• Application for Retirement
• Retirement Benefit Election Form (to continue insurance)
• W4P Federal Income Tax Form • Direct Deposit Form
• Retiree Beneficiary Form
• Final Pay-457 (B) Rollover Form
2. Please attach a copy of your birth certificate and a voided check with your application.
3. Your retirement application is submitted to the Pension Board for approval. The Board meets on the 4th Thursday of each month.
4. After the Pension Board’s approval, your application packet with the approval retirement percentage and resolution is submitted to the Pension Payroll department for the calculation and set up of your semi-monthly pension benefit.
5. Please keep in mind that your semi-monthly pension benefit cannot be calculated and set up until you receive your final check and you have been separated from the active payroll. the separation from payroll documents, reflecting final pay, must be initiated, approved, and submitted to Human Resources by your Division Director.
6. You should receive your first semi-monthly pension benefit within approximately 60 days after your Final Pay and will include any retroactive payments You have the option to receive a one-time lump sum payout of your contributions in lieu of receiving a semi-monthly benefit. 7. Final Pay Rollover to your Deferred Comp 457(B)-You can make contributions up to the maximum IRS Limit- Complete sections A and B (Deferred Salary Contributions), sign, and date on page 2.
