Life Event Type and Required Documentation
Marriage:
-Marriage License
-Social Security Card
-Letter from Spouse's Job (on company letterhead) stating that insurance is not offered.
-Complete the spousal verification form and the active enrollment change forms below.

1. Please complete the enrollment change form listed below and submit the form along with the above documents to the Benefits Department via the benefitsquestions@memphistn.gov email address.
